I certify that I intend to continue my studies at Touro Law Center in Fall 2013. I understand that my acceptance of this FWSP award precludes me from applying to transfer to another law school and that, if I do, my violation of the terms of this agreement will be considered a violation of the Law Center’s Code of Conduct and may be reported to the relevant authorities in any state where I seek admission to the bar. I understand, further, that if, for any reason, I do not return to Touro Law Center for the 2013-2014 academic year, I will pay back the entire amount of this FWSP award (as stipulated in the Touro FWS Program Agreement) to Touro Law Center by September 20, 2013.

I certify that I am in good academic standing (2.333 GPA), as of the end of the Fall 2012 semester.

I certify that I understand Federal Work Study Program (FWSP) funding is awarded on a first-come, first-served basis. This form must be submitted no earlier than 12:00 AM, Wednesday, February 6, 2013 but no later than 05:00 PM, Friday, May 31, 2013.

I certify that I understand in order to be considered for Federal Work Study Program (FWSP) funding, I must also complete the 2013-2014 Free Application for Federal Student Aid (FAFSA). It is advisable that students complete the FAFSA two weeks after filing their federal tax return electronically or four weeks after submitting a paper copy to the IRS. Commencing 2013, income data on the FAFSA will be retrieved and verified by the IRS. Discrepant information will cause significant delays in processing. When ready to complete the FAFSA go to: www.fafsa.ed.gov. The FAFSA school code is: 010142.

I certify that in order to maintain my position on the FWSP funding roster, I must have a firm offer from a not-for-profit organization or a government agency by no later than April 29, 2013.
